1. Application
2. New Pupil Registration
3. Record Release Form
4. Transfer form (yellow slip from public school)
5. Transfer form (from Charter School)
6. Medical History Form

The following documents must be submitted:

1. Birth Certificate
2. Immunization Record
3. Proof of Residency (PSE&G OR Telephone Bills )
4. Valid/Most Recent Report Card
5. IEP (for special education students only)
